Expanding our Impact

 At Tree of Hope, our mission has always been to help individuals achieve safe, successful, and sustainable community living. As the needs of the communities we serve continue to grow, Tree of Hope is expanding our services and geographic reach to better support individuals and families. 

 Tree of Hope is actively working to expand services into Minnesota, bringing our person-centered approach to transition coordination and community-based support to additional individuals seeking independence and quality of life. Our expansion into Minnesota reflects our commitment to providing compassionate, high-quality services that empower individuals to successfully transition and thrive in their communities. As we continue this growth, we remain dedicated to building strong partnerships with healthcare providers, community organizations, housing agencies, and support networks in both Colorado and Minnesota. 

 In addition to our Transition Coordination services, we are providing Home Health Care needs through Colorado HCBS Services. This expansion will allow us to provide a broader continuum of support services designed to help individuals remain safe, independent, and successful in their homes and communities. Our goal is to create a seamless experience for individuals transitioning from institutional settings by connecting them with the services and supports they need to maintain long-term stability and independence.
