STEP ONE:
INITIAL OPTIONS COUNSELING
Medicaid Transition Services Options Counseling is a program that provides options counseling for Medicaid members living in skilled nursing facilities, regional centers and intermediate care facilities. Options counseling agencies help long-term residents of local skilled nursing facilities and other qualified facilities learn what options might be available for community living.
Our Transition Services team will work with you to choose an options counseling agency and get enrolled to begin your transition.
STEP TWO:
MEETING WITH YOUR TRANSITION TEAM
Once we receive your referral form from the options counseling team, we will reach out to you to set up an initial meet and greet to talk about the program.
STEP THREE:
YOUR HOUSING VOUCHER, AND HOUSING SEARCH
Our transition services team will work with the Department of Housing to review your information to determine if you qualify to move forward with their housing navigation services. They will review your financial situation as well as possible past evictions.
You will take a housing orientation class that will go over details regarding housing support.
STEP FOUR:
MOVING IN
Medicaid provides a small stipend for the Transition Agency to buy furniture, household necessities and groceries for a successful living situation.
Our team will help collect furniture and assist in moving it into the apartment, assisting in getting you all set up for your new chapter in the community.
STEP FIX:
LIVING INDEPENDENTLY IN THE COMMUNITY
We work with several Home Health Agencies to help you complete your HCBS waiver services and help in finding you a caregiver to make sure you continue to have in home support that is vital to your success.
