Our Services

 Tree of Hope Transition Coordination Agency provides person-centered, community-based services designed to support individuals through every stage of transition—from facility-based care to stable, independent living in the community.

Our services focus on reducing barriers, coordinating care, and ensuring each member has the tools and support needed for a successful and sustainable transition.

Transition services

 Our Transition Services are designed to support individuals moving from institutional settings into safe, stable community living. We provide hands-on coordination throughout the entire transition process, ensuring each member has a clear, supported pathway back to independence.

We assist with discharge planning, housing coordination, benefits navigation, and connection to essential community resources. Our team works closely with members, families, and care providers to ensure transitions are safe, timely, and person-centered.

Our Transition Services include:

  • Comprehensive needs assessment and transition planning 
  • Skilled nursing facility (SNF) and hospital discharge coordination 
  • Housing navigation and community placement support 
  • Benefits and waiver program assistance 
  • Connection to community-based services and providers 
  • Barrier resolution and ongoing case coordination 
  • Post-transition follow-up and stabilization support 

Our goal is to ensure each individual has a supported and successful return to the community with the right services in place.

 We receive referrals from nursing facilities, hospitals, Options Counseling agencies, healthcare professionals, family members, and community partners. Our team reviews eligibility requirements and begins the transition planning process.

 We complete a comprehensive assessment and develop a personalized transition plan that addresses housing, healthcare, transportation, benefits, and support needs.

 Our team assists with housing applications, voucher navigation, community resources, furnishings, and other services necessary for a successful transition.

Tree of Hope provides In-Home Support Services (IHSS) and Home Health services to support individuals in maintaining independence and stability within their home and community settings.

These services are designed to extend support beyond transition, ensuring members continue to receive the assistance they need after discharge or community placement. Services are tailored to each individual’s needs and care plan.

  • Assistance with activities of daily living (ADLs) 
  • Personal care services 
  • Homemaker and daily living support 
  • Medication reminders and health routine support 
  • Coordination of home-based care services 
  • Support with ongoing community living stability 

We work collaboratively with members, families, and care teams to ensure services are person-centered, goal-driven, and aligned with long-term independence and quality of life.

Colorado Medicaid Waiver Services

Tree of Hope provides support and coordination for individuals enrolled in Colorado Medicaid Home and Community-Based Services (HCBS) waiver programs. These waivers are designed to help individuals receive services in their home or community rather than institutional settings.

We assist members and families with understanding waiver eligibility, navigating the application process, and coordinating services once approved. Our team works closely with case managers and providers to ensure services are aligned with each individual’s needs and transition goals.

 We commonly support members enrolled in the following Colorado waivers:


  • Community First Choice (CFC)
  • Elderly, Blind, and Disabled (EBD) Waiver 
  • Community Mental Health Supports (CMHS) Waiver 
  • Brain Injury (BI) Waiver 
  • Supported Living Services (SLS) Waiver 
  • Children’s Extensive Support (CES) Waiver 
  • Developmental Disabilities (DD) Waiver 

Our waiver-related support includes:

  • Assistance with waiver enrollment and eligibility navigation 
  • Coordination with case management agencies 
  • Service planning and linkage to approved providers
